Salary: $27 to $29 per hour

Position Summary: The role of the Lead Hospice Aide, under the supervision of the Hospice Aide Manager, is to assist with timely coordination, scheduling, and/or completion of patient visits. The hospice aide is responsible for assisting terminally ill patients with their personal care needs, ensuring the highest level of comfort and quality of life, consistent with their health status, wherever the patient lives. Aide services are assessed by a registered nurse every 14 days and as needed, to assure that the aide demonstrates satisfactory performance.

Qualifications:
  • Listed on MN nursing assistant registry with current Nursing Assistant Certification.
  • 1-year experience as a home health aide or nursing assistant.
  • Current valid MN Driver’s License and daily access to reliable, insured vehicle.
  • Ability to use and/or learn electronic charting on tablet and/or mobile phone.
  • Demonstrates a desire to work with the terminally ill.
  • Ability to demonstrative positive and respectful communication/interaction skills.           
Job Duties:
  • Coordinates scheduling, planning, and coverage for hospice aide visits, ensuring continuity of care during staff absences or periods of increased demand.
  • Provides direct personal care to patients as outlined in the individualized plan of care.
  • Serves as a clinical and operational resource for hospice aides, offering guidance, support, and mentorship as needed.
  • Demonstrates professionalism, courtesy, and respect in all interactions with patients, families, staff, community partners, and referral sources.
  • Actively seeks, accepts, and incorporates feedback to improve job performance and team collaboration.
  • Maintains strict patient confidentiality and adheres to all HIPAA regulations and organizational policies.
  • Demonstrates knowledge of and sensitivity to vulnerable adult issues, ensuring patient safety and dignity at all times.
  • Upholds and promotes hospice patients’ rights in accordance with regulatory and organizational standards.
  • Completes all required documentation accurately and in a timely manner, in compliance with agency standards.
  • Exhibits a strong commitment to providing compassionate care to individuals with terminal illness.
