Position Summary: The Intake & Admin Support Coordinator is a flexible and knowledgeable team member who supports both the Intake and Clinical Administrative Support functions within the organization. This position serves as a vital resource, stepping in as needed to assist with patient intake, referral management, documentation processes, and interdisciplinary group (IDG) meeting coordination. The ideal candidate demonstrates a strong understanding of hospice operations, excellent communication skills, attention to detail, and the ability to shift between administrative and clinical support tasks seamlessly.

Salary: $27.00 to 30.00 per hour

Job Duties:
  • Provides support to the Intake team by completing patient referrals, scheduling admissions, consults, and assessments
  • Communicates effectively with families, caregivers, and referral sources regarding hospice services and the Medicare Hospice Benefit
  • Accurately enters patient data and documentation into the electronic health record in real time
  • Assists in processing admission and discharge paperwork, including use of DocuSign
  • Supports Clinical Admin team by preparing and distributing IDG meeting documents and performing post-IDG follow-up
  • Processes verbal orders, Plans of Care (POCs), and Certifications of Terminal Illness (CTIs)
  • Submits medication reviews to Pharmacy Benefit Manager (PBM) and client records to DHS as needed
  • Provides phone support and serves as back-up for both Intake and Clinical Administrative teams as needed
  • Participates in staff in-services and daily morning intake/referral meetings
  • Maintains a proactive, team-based approach to departmental support and coordination
  • Other duties as assigned
Qualifications:
  • Business College Degree or 2-4 years office experience
  • Ability to type 65 words per minute
  • Working knowledge of computer hardware and software
  • Ability to read, write, speak and comprehend English
  • Excellent oral and written communication skills
  • Ability to prioritize and organize multiple tasks
  • Ability to work independently
  • Excellent interpersonal skills
  • Ability to respect confidentiality
