Job Overview:
The Patient Care Manager (PCM) position serves as a leader, providing supervision for direct care staff. This position serves as a management position with both clinical and management responsibility for their team and serves as a liaison to and conduit between the Medical Director, NPs, DPC, regional RNs, LPNs, Resource RNs, patients and their families, and the community. The PCM participates in the implementation of clinical policies and procedures and carries out the organization’s purposes in a manner consistent with the goals, objectives and established practices.  Ability to provide case management, visit assistance, and hospice admissions as needed.

Key Responsibilities:
  • Lead regional team of RNs, LPNs and Resource RNs in providing exceptional patient care
  • Facilitate morning call for regional team, addressing coverage needs and supporting the care teams in meeting patient care needs daily
  • Supervise, mentor and develop assigned RNs, LPNs, and Resources RNs
  • Ensure high-quality, compassionate care is delivered according to POC, oversee care coordination and quality assessments
  • Collaborate with all departments
  • Assist in the recruiting, selecting, hiring and orienting of qualified individuals
  • Provide assistance in the field as needed, shadowing and supporting direct reports as needed
  • Manage team staffing needs
  • Supervisor on-call responsibilities
  • Support team through development of programs, goals and objectives consistent with those of the organization’s overall strategic plan and initiatives
  • Actively participates in leadership meetings, collaborating and communicating with leadership team
  • Facilitate regularly scheduled team meetings to create and maintain a cohesive team approach in providing excellent end of life care
  • Prepare for and lead bi-weekly IDG meeting
  • Responsible for building and maintaining employee morale, motivating employees to reach their full potential including professional development
  • Assist in overseeing compliance of clinical charts with regular clinical reviews and audits
  • Perform clinical administrative tasks
  • Uses available resources and data to guide excellent patient care
  • Assist in budget process and staffing plans
  • Support and participate in quality improvement
  • Identify and act upon improvement opportunities for the overall success of Grace Hospice
  • Other duties as assigned
Qualifications:
  • Previous supervisory experience
  • Current Minnesota RN licensure
  • Associate or Bachelor’s degree
  • Must have the ability to direct and supervise staff and lead in a professional manner
  • Current Minnesota Driver’s License, insurance and daily access to reliable vehicle
  • Minimum of 3 years’ hospice, long term care or community health experience
  • Excellent written and verbal communications skills
  • Excellent with relationship development internally and externally
  • Ability to think critically
  • Ability to network within the referral communities
  • Strong knowledge of hospice regulations including CMS, state licensure and conditions of participation
  • Ability to respect confidentiality
