Grace Hospice is looking for a compassionate and adaptable Hospice Resource RN to join our care team.
This is a great opportunity for a nurse who enjoys a dynamic role and being a trusted resource when patients, families and clinical teams need support. As a Resource RN, you’ll complete hospice admissions, provide patient visits as needed, and work closely with Intake, Clinical Managers, RN Case Managers and the interdisciplinary team to help ensure smooth transitions and coordinated care.

What You’ll Do
- Complete hospice admissions and help patients and families feel informed and supported as they begin hospice care.
- Provide follow-up and patient visits based on changing care needs.
- Collaborate with Intake, Clinical Managers, RN Case Managers and the interdisciplinary team to coordinate care and schedules.
- Respond to patient, family and facility requests for clinical support.
- Assess and communicate changes in patient condition or needs to the appropriate members of the care team.
- Obtain physician orders and complete required documentation in the electronic health record.
- Adapt to changing patient and team needs while helping maintain continuity of care.
- Graduate of an accredited school of nursing.
- Current Minnesota RN license.
- Three years of recent nursing experience in acute care, long-term care, home care, hospice or public health preferred.
- Strong assessment, care-planning and patient-education skills.
- Excellent communication, critical-thinking and problem-solving abilities.
- Comfortable working independently while collaborating closely with a clinical team.
- Experience using electronic health records and standard computer applications.
- Valid Minnesota driver’s license, auto insurance and daily access to reliable transportation.
