Position Summary

The Intake Specialist is one of the first points of contact for patients, families, caregivers, and referral sources seeking hospice services. This position requires a basic understanding of the Medicare Hospice Benefit and the ability to explain hospice services clearly, compassionately, and patiently.

The Intake Specialist coordinates admissions, consultations, assessments, and staff schedules through phone calls, text messages, and email. This position is also responsible for entering and maintaining accurate, timely information in the electronic health record.

Compensation

$26.00–$30.00 per hour, based on experience and qualifications.

Qualifications

  • High school diploma or GED required

  • Associate degree in business, healthcare administration, or a related field preferred

  • Two to four years of administrative or office experience preferred

  • Healthcare, hospice, home care, or medical scheduling experience preferred

  • Ability to type at least 65 words per minute

  • Working knowledge of computer hardware, software, email, and electronic records

  • Ability to read, write, speak, and understand English

  • Excellent verbal and written communication skills

  • Strong organizational and time-management skills

  • Ability to prioritize and manage multiple tasks

  • Ability to work independently and as part of a team

  • Excellent interpersonal and customer-service skills

  • Ability to maintain confidentiality and handle sensitive information appropriately

Essential Job Duties

  • Receives and processes patient referrals and intake information

  • Schedules admissions, consultations, and assessments

  • Communicates proactively with patients, families, referral sources, and clinical team members

  • Enters accurate and complete information into the electronic health record in real time

  • Ensures patient information is available to on-call staff and other members of the care team

  • Coordinates schedule changes as they occur

  • Scans, faxes, emails, and organizes documents as needed

  • Participates in daily intake and referral calls

  • Participates in staff meetings, in-services, and training programs

  • Performs additional administrative duties as assigned
