Hospice Social Worker, On Call Weekends

Position Summary: Hospice Social Worker On Call Weekends.  As part of the hospice team you will provide psychosocial support, complete admissions to hospice, and make visits as needed.  You will be an important member of the team and communicate your findings and cares with the team.  You must possess expertise in end of life care and a persona that includes kindness and compassion.

Job Requirements:

  • Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources.
  • Functions as a team member with other staff in the performance of duties.
  • Seeks and accepts feedback regarding the performance of job duties.
  • Is aware of patient confidentiality and adheres to HIPPA regulations.
  • Displays an understanding of vulnerable adult issues. 
  • Displays an understanding of hospice patients’ rights.
  • Exhibits sound mature judgment.

Qualifications:                                 

  • Masters in Social Work preferred, from an accredited program and licensure in the state of Minnesota.
  • Social Work experience with individuals and families, end of life and medical Social Work experience preferred.
  • Knowledge and experience with family systems theory, individual and family counseling, and group process.
  • Ability to work independently, takes initiative, and thinks creatively.
  • Knowledge of community resources including Medicare/Medicaid, including the hospice benefit, county, state and private social service agencies.
  • Critical thinker. 
  • Ability to work well with an interdisciplinary team.
  • Excellent written and verbal interpersonal communication skills.
  • Current Minnesota driver’s license, insurance and reliable daily transportation.
