Position Summary: The Licensed Practical Nurse (LPN) is a medical professional who provides compassionate end-of-life care to terminally ill patients. The LPN will function as part of an interdisciplinary team under the supervision of the Patient Care Manager and will perform patient visits as assigned by the RN Case Manager. The LPN will play a vital role in the coordination of care of the patient to ensure their loved ones, facility partners, and hospice team are aware of any changes or needs.

- Patient Care Delivery
- Provide direct patient care to terminally ill patients; identify and address their individual physical, psychosocial, safety, and comfort needs as directed by the care plan and RN case manager
- Confident management of chronic and acute end-of-life symptoms and medications
- Provide wound care, personal hygiene and assist with feeding as needed
- Train/educate patient, family, caregivers, and partner facilities on end-of-life care
- Collaborate with the interdisciplinary team and others involved in the patient’s care
- Required participation in bi-weekly IDG meetings
Qualifications:
- Graduate of an accredited vocational school of nursing
- Holds a current Minnesota LPN license
- Previous nursing experience is preferred
- Strong interpersonal communication and collaboration abilities; both verbal and written to interact with patient, families and facility staff
- Competency in electronic patient and hospice management systems
- Current MN Driver’s License, insurance and daily access to reliable vehicle
- Ability to critically think
