RN Weekend Admission Nurse – Home Health

At Grace Home Health and Hospice we offer professional skilled care with a personal touch by focusing on the patients and families, not just the disease.  We have built our nonprofit Home Health Agency to look holistically at each individual person and provide support to help them remain independent and safe in their own home.


Provide admission and follow-up skilled nursing visits for home health patients.

Consult with physician in developing the initial patient care plan based on physician's orders and initial patient assessment. 

Complete patient assessment, hands-on care, teaching of the patient in accordance with physician orders.

Coordinate appropriate care, including various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).

Report patient care/condition/progress to patient's physician and Clinical Manager.  

Develop the patient care plan in conjunction with patient and family to assist them in achieving optimal results of needs/problems. 

Prepare appropriate medical documentation on all patients, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. 

Document patient information in electronic medical record timely and accurately.

Coordinate care with other health care professionals as necessary.

Is aware of patient confidentiality and adheres to HIPPA regulations.
Displays an understanding of vulnerable adult issues. 
Displays an understanding of home health patients’ rights.
Exhibits sound mature judgment.
Patient Care Delivery


  • Graduate of accredited school of nursing
  • Current state of Minnesota RN licensure
  • 1 year recent nursing experience in acute, long-term care or home care/hospice/public health
  • Strong patient assessment, care planning and patient teaching skills
  • Excellent written and verbal communication skills
  • Critical thinker
  • Current Minnesota driver’s license, insurance and daily access to reliable vehicle
  • Computer literacy
