Position Summary:  The Hospice Supervisor position serves as a leader, providing supervision for direct care staff. This position services as a management position with both clinical and management responsibility for his/her team and serves as a liaison to and conduit between the Hospice Director, Patient Case Manager, clinical staff on his/her respective team, patients and their families, and the community. The Hospice Supervisor participates in the implementation of clinical policies and procedures, and carries out the organization’s purposes in a manner consistent with the goals, objectives and established practices.

Job Requirements

Interpersonal Relationships

  • Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources
  • Functions as a team member with other staff in the performance of duties.
  • Seeks and accepts feedback regarding the performance of job duties.
  • Is aware of patient confidentiality and adheres to HIPPA regulations.
  • Displays an understanding of vulnerable adult issues.
  • Displays an understanding of hospice patients’ rights.
  • Exhibits sound mature judgment.
  • Trains and teaches procedures, policies, etc. 
  • Assesses, develops, evaluates and implements clinical service operations.
  • Responsible for hiring, mentoring and orientation of licensed staff.
  • Responsible for performance management of licensed staff.

Patient Care Delivery

  • Collaborates with the RN Case Manager, other hospice team members as appropriate and the attending physician to coordinate the delivery of patient care.
  • Reviews admission and all higher level of care including respite, general inpatient, and continuous care.
  • Is available on-call.
  • Ability to step in as RN Case Manager as needed.
  • Trains and teaches procedures, policies, etc.
  • Participates in Quality Assurance program.

Supervisory Tasks:                        

  • Serves as a role model and a resource for nursing staff and other members of the health care team.
  • In collaboration with Hospice Director, determines the hiring, mentoring and retention of the hospice team members.
  • Responsible for hiring, mentoring and orientation of licensed staff.
  • Responsible for performance management of licensed staff.
  • Delegates tasks based on professional judgment
  • Responsible for special projects as assigned by the Hospice Director.
  • Participates in IDG.

Qualifications:                                

  • Previous supervisory experience
  • Graduate from accredited school of nursing for Registered Nurse
  • Current state of Minnesota RN licensure
  • Must have the ability to direct and supervise clinical staff and lead in a professional manner
  • Current Minnesota Driver’s License, Insurance and daily access to reliable vehicle
  • Written and verbal communication skills
  • Critical thinker
  • Minimum of 3 years’ hospice, home care or community health experience
  • Computer literacy
